Most men with early prostate cancer have no symptoms. Minor voiding symptoms may be related to associate benign enlargement of the prostate than the early prostate cancer. More locally advanced prostate cancer may cause significant voiding symptoms or urinary retention and or obstruction of the ureters and renal failure. Prostate cancer that has spread to the bones may cause pain or fracture of the vertebrae with spinal cord compression and paralysis. HIFU stands for High Intensity Focused Ultrasound and is delivered via a transrectal probe using simultaneous ultrasound imaging during anesthesia. The sonablate technology uses high intensity ultrasound generated by powerful transducers which is then focused to raise the temperature at the focal point to almost 90 degrees. Any tissue at the focal point within the prostate is then destroyed through thermal coagulative necrosis.
Testosterone profoundly affects the development of prostate cancer. Prostate cancer does not occur in eunuchs. There is an increased incidence of prostate cancer amongst relatives of patients with prostate cancer and in Afro-American men. The HIFU for Prostate Cancer along with a high fat diets inflammatory cell within the prostate may lead to cells which may be precursors of prostatic intraepithelial neoplasia and prostatic carcinoma. Tissue outside the focal point is unharmed. Using enhanced software the focal point is programmed to move so as to create a confluence of heated areas so that all the targeted tissue within the prostate is destroyed through this process of thermal coagulative necrosis. This minimally invasive treatment is performed as an outpatient and on average takes 1 to 4 hours depending upon the size of the prostate.
